This article reviews current prevention and treatment strategies according to international (ESMO, MASCC, NCCN) and Russian recommendations. The mechanisms of tissue injury are discussed for different drug classes, including anthracyclines, vinca alkaloids, taxanes, and platinum agents. A step-by-step algorithm and detailed table of antidotes (dexrazoxane, hyaluronidase, sodium thiosulfate, DMSO) are presented. The article highlights practical implementation issues in Russian clinics, where lack of standardized extravasation kits and antidotes remains a problem. Early recognition, staff training, and safety culture are key to reducing complications.</p> <p> </p></abstract><trans-abstract xml:lang="ru"><p>Экстравазация цитостатиков – редкое, но потенциально тяжелое осложнение противоопухолевой терапии. В статье представлен современный подход к профилактике и лечению экстравазации с учетом международных рекомендаций (ESMO, MASCC, NCCN) и российских реалий.
